Individualized Care Planning

Our Approach
Care That Starts With Listening
Before any care begins, we will take time to understand the prospective resident as a person — their history, preferences, routines, abilities, and what matters most to them and their family.
Care plans will be developed collaboratively and reviewed regularly to ensure they continue to reflect each resident's evolving needs and goals.
We will maintain open communication with families and authorized representatives, and we welcome questions and involvement throughout the care relationship.
Who We Serve
Finding the Right Home Matters
Choosing residential care is an important decision. Rainier Care Homes welcomes conversations with individuals, families, case managers, social workers, discharge planners, and other professionals looking for an appropriate residential care setting.
Because every person's needs are different, prospective residents will be evaluated individually to determine whether our home can safely and appropriately meet their needs.
